After its release from the thyroid thyroxine (T4) is converted to triiodothyronine (T3) on demand. T3 is considered the most active thyroid hormone. T4 is also converted to reverse triiodothyronine (RT3) which is far less active and is often called the ‘hibernation hormone’.

The ratio of T3 to RT3 depends on the body’s metabolic needs. The reason is, T3 and RT3 are used to speed up, or slow down metabolism. In a clinical setting high reverse T3 (RT3) indicates you are converting most of your T4 into RT3.
